Preparing for your appointment at Mid-Delta Health Systems is easy. Here’s what you need to do:

Gather Your Information:

Make sure to bring your insurance card, identification, proof of income, and a list of your current medications.

Arrive Early:

We recommend arriving 15 minutes early to fill out any necessary paperwork. If you prefer, you can also submit registration forms online.

Bring Past Medical Records:

If you have been seen at another facility, we request that you provide us with your medical records to help us provide you with the best possible care.

Our automated phone system will call you one week before your appointment to remind you of your upcoming visit. Our front desk team will also call you the day before to confirm your appointment and answer any questions you may have!

Good Faith Estimate

Standard Notice: "Right to receive a good faith estimate of expected charges" under the no surprises act

You have the right to recieve a “good Faith Estimate” eplaining how much your medical, mental health care or dental services will cost.

Under the law, health care providers are required to give patients who do not have insurance or who are not using insurance, an estimate of the expected charges for services. 

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services. 

You can ask your health care provider, and any other provider you choose for a Good Faith Estimate before you schedule a service.

If you receive bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a  copy or picture of you Good Faith Estimate.
